Survived by her parents, Dan Dena (Hornak) Ford; maternal grandparents, Mike Karen Hornak; paternal grandparents, Chuck Sue Stiles; sisters, Charlotte Rae Ford, Brooklyn Marie Ford; several aunts, uncles, and cousins. Special thanks to the caregivers working in the NICU at Strong Memorial Hospital. Avery's funeral service is Monday at 10 AM at the funeral home, 1411 Vintage Lane (between 390 Long Pond Rd.)
